are requesting a proposal (attached is a
list of services we anticipate needing over the next year)
from you that outlines potential services and business information
as one of our vendors, to include (if applicable):
A
cost list of services and/or supplies based on unit prices
Areas
and regions where your services are available
Hours
of availability, including week days, evenings, weekends and
holidays
Proof
of Insurance – Please see attached
requirements
Business
License and Contractor’s License, if applicable.
Title
51 Waiver – Please note that a
contract with LIHI will require a lien indemnification as well.

will be requiring a minimum 1 year service contract with new
vendors. Please indicate
