To receive a housing pre-application, stop by LIHI at 2407 1st Avenue in downtown Seattle, call LIHI at (206)443-9935 to have a housing application mailed or faxed to you, or select the application below for a Microsoft Word version, after reading the following instructions.

Pre-Application Form
All LIHI properties located in Seattle now use a single pre-application form.  This form contains basic information that allows us to determine which buildings you are currently eligible for and how to contact you.  Please be advised, once your name does come up on the waiting list, you will be required to fill out a comprehensive application and provide up to date documents that determine your eligibility.

For LIHI properties located outside of the Seattle city limits please contact the main office for application instructions.

The Pre-Application Form must be fully completed and signed, then returned to LIHI in order to apply for housing.

Please send or bring the completed pre-application form to the LIHI office, and we will distribute your pre-application to each site you qualify for.  Deliver or mail the application to:

All housing owned and/or operated by LIHI is located in the Puget Sound area in Washington State.

Once the site has received your application, you will receive a postcard with information about your place on the waiting list.  Please keep this information for your records.

Waiting lists are kept separately at the different sites, not at the LIHI office.  You may be on several waiting lists at the same time.  It is your responsibility to update your contact information.  If your address, phone number, income, or household size changes, send the new information in writing to the LIHI office.  We will copy this information and forward it to the appropriate sites.

When your name comes up on the list, we will contact you by phone or mail.
